What is Phimosis?

Phimosis is a condition in males in which the foreskin doesn’t pull back from the tip of the penis. Every boy is born with a tight foreskin. With age, the foreskin starts to retract and by the time they turn 3, this no longer remains a problem as the foreskin completely loosens up. This is a common problem in young boys which usually gets cured on its own. But in severe cases, when urination becomes difficult or pain is severe, treatment becomes essential.

    Treatment of Phimosis

    Diagnosis of Phimosis

The diagnosis of phimosis is a simple one. The urologist makes note of the full medical history of the patient by asking questions regarding any prior infection or injuries to the penis. There can be some questioning pertaining to any impact that might have occurred during sexual activity. Followed by the questioning round, the doctor does a physical examination in which he thoroughly looks at the penis and foreskin. To confirm, the doctor recommends a urinalysis to check for any urinary tract infections or perform a swab test by taking a sample from the foreskin area to check for bacteria.

    Surgery

The surgical removal of the foreskin is known as circumcision. In this procedure, the urologist frees the foreskin from the penis’s head and the extra skin is removed. The benefits of circumcision are discussed below-

    • Lower risk of urinary tract infections
    • Lower chances of sexually transmitted diseases in men
    • Decreasing the risk of penile cancer in males and cervical cancer in females
    • Preventing conditions such as balanitis and balanoposthitis
    • Preventing phimosis and paraphimosis

    Frequently   Asked Questions

Yes phimosis is curable. Circumcision is the procedure to treat phimosis once and for all. Laser circumcision is the safest way to remove the tight foreskin from the penis. ZSR circumcision is a modern painless technique of circumcision that is minimally invasive in nature. It is better to undergo any of these advanced circumcision procedures than complex open circumcision surgery.

If you have a tight foreskin, do not retract it forcibly. This can result in tearing of the foreskin tissues and can aggravate your problem. Also, the foreskin may get stuck and form a ring around the penis. This condition is called paraphimosis and is an emergency medical condition.

Although phimosis can be treated with ointments and medications, the only permanent treatment for the condition is the surgical procedure called circumcision. In circumcision, the surgeon removes the layer of foreskin from the penis, which resolves the problem of phimosis. At present, the most effective and modern circumcision procedure is laser circumcision.

Phimosis in infants usually goes away on its own by the time the boy turns 7 years of age. However, cases of adult phimosis do not resolve on their own without medical treatment. If left untreated, they can cause a number of complications. Therefore, it is better to seek medical consultation for the problem.

Yes, inability to retract the foreskin can make it even more difficult to maintain proper hygiene of the penis. Microbes can thus thrive in the dark and moist area under the foreskin and cause infections, which can result in inflammation of the glans penis.

Yes, phimosis does occur in children. In fact, infant phimosis is very common and occurs due to adhesions between the foreskin and the glans penis. The problem is nothing to worry about and it usually resolves on its own by the time the child reaches 5-7 years of age.

Sexually transmitted infections, improper hygiene and skin conditions like eczema and psoriasis are the major causes of sudden phimosis in adult men.

No, you should not rely on self-medication and home remedies for phimosis. Home remedies can certainly help alleviate the discomforts of phimosis only temporarily. However, it is not a good idea to rely completely on home remedies. And self-medication is just not an option. Delay in medical treatment for phimosis can result in the worsening of the problem and the symptoms can aggravate.

To treat phimosis at home, the doctor often suggests foreskin stretching exercises. In case stretching does not help, the doctor may recommend alternative medical treatments and home remedies before resorting to surgery, such as:

Antibiotics - If phimosis is the result of bacterial or fungal infection, antibiotics are prescribed to kill the infection. Common antibiotics prescribed to treat phimosis are clotrimazole and flucloxacillin.

Phimosis creams - Doctors often suggest steroid creams or ointments for phimosis in mild cases.

However, home treatment for phimosis may not resolve the condition effectively or permanently if the condition is severe.

Good hygiene of the penis can lower the chances of phimosis or other related conditions. Some good penis hygiene tips are described below-

Wash the foreskin regularly. The foreskin should be pulled back and gently rinsed with soap and water every time a bath is taken. This is done to prevent any buildup of urine, dirt, bacteria, and other substances.

    • The entire penis- tip, shaft, base, and scrotum- should be washed thoroughly.
    • Wear loose, breathable underwear so that excessive moisture doesn’t build up under the foreskin.
    • Pubic hair should be removed to prevent bacterial/fungal infections that can lead to phimosis.

Yes, you can have sex with phimosis. But you may experience pain and discomfort during sexual intercourse. The tight foreskin also affects the chances of achieving orgasm pleasurably. You will also have to be cautious to prevent any tearing or scarring of the foreskin. It is better to consult a doctor for a permanent solution for phimosis.

It is not at all advisable to leave phimosis untreated. Prolonged untreated phimosis can cause swelling, inflammation, and sharp pain in the penis. In severe cases, it can even result in the death of penile tissues due to loss of blood supply. Hence, get in touch with an experienced urologist as soon as you notice the symptoms of phimosis.

Circumcision is the surgical removal of the foreskin of the penis. Generally, a general surgeon or a urologist performs the circumcision. In the case of circumcision in infants, a pediatrician can perform the surgical procedure.

Surgicare is one of the best circumcision clinics. Some of the top urologists work with Surgicare to provide treatment for diseases such as phimosis, paraphimosis, and balanitis. At Surgicare, circumcision is done through laser and stapler technology.

Circumcision is generally performed as a daycare procedure which means the patient can return home the same day. The patient may need to stay at the hospital only if he has any underlying health issue that may lead to post-surgical complications. If everything seems fine with the patient's health, he can return home just after few hours of the surgery.

The cost of circumcision primarily depends on the reason. If it is done for a religious or cultural reason where there are no underlying health issues, the treatment can be done at a relatively lower price. If circumcision is done to treat any medical issues or penile-related problems, the cost may go higher. Usually, in either case, the price should range somewhere between INR 25000 and INR 40000.

Circumcision is a relatively safe surgical procedure that can be completed within 30 minutes. In this surgery, the urologist or the general physician removes the foreskin of the penis with high precision either through the open, laser, or stapler method.

    Is phimosis dangerous?

If identified and treated at an early stage, phimosis does not pose any major threats to the life of any man. But if taken lightly and not provided with treatment, tight foreskin can be dangerous. Men with tight foreskin are at a greater risk of developing penile cancer. If left untreated, phimosis can result in the inflammation of the foreskin, glans penis or both.
Read MoreHence, it is very important for a man to maintain good penile hygiene and be gentle while cleaning the foreskin to prevent pathological phimosis. Additionally, men should avoid using harsh soaps and cleansers to clean the genitals and use only lukewarm water to clean the prepuce.

Are phimosis and paraphimosis the same?

Phimosis and paraphimosis are both common foreskin problems but are different from each other. In phimosis, the prepuce becomes tight and cannot be retracted over the glans penis. It is very common in babies and young boys. In older men, the problem may occur due to chronic penile infection and inflation in the foreskin and the penis. The tight foreskin also causes problems in urination and any sexual activity as well. On the other hand, in paraphimosis, the foreskin can be retracted but cannot be pulled forward again. It mostly occurs if the foreskin is retracted immediately after a medical procedure. Paraphimosis leads to swelling of the glans penis and increases pressure to the retracted foreskin. The stuck foreskin forms a constricted ring around the penis. The condition prevents the flow of blood from reaching the penis, which may cause damage to the penile tissue. While phimosis is not a medical emergency, paraphimosis is. Paraphimosis needs medical treatment at the earliest

When do you need circumcision for phimosis?

Doctors recommend circumcision in case of severe or recurring phimosis. It is the surgical procedure to remove the layer of the foreskin from the penis. In addition to treating various foreskin problems like phimosis, paraphimosis, etc, circumcision offers other medical benefits too.

    Benefits of circumcision

Medical practitioners consider circumcision to be highly effective for penile problems like phimosis. The primary benefits include:
    • Reduced risk of STIs in men
    • Protection against penile cancer
    • Lesser chances of urinary tract infections
    • Prevents smegma build-up
    • Lowers the risk of cervical cancer in their female sexual partners
    • Prevents balanitis (inflammation of the glans penis) and balanoposthitis (inflammation of both foreskin and the glans)

    Risks of circumcision

Like any other surgical methods, circumcision too has some amount fo risks including:
    • Post-surgery pain
    • Risk of swelling and infection in the surgical area
    • Risk of penile injury
The benefits of circumcision outweigh the risks when performed precisely. If performed by an experienced doctor, the patient is highly unlikely to suffer from any risk or complication. Furthermore, doctors nowadays use advanced laser technique for circumcision. In this procedure, only laser energy is used to remove the prepuce without damaging the neighboring tissues.

Following are the advantages of laser circumcision over open surgery:

    • Non-invasive procedure
    • Completes within 15-20 minutes
    • Painless procedure- with the use of anesthesia
    • Outpatient procedure
    • No stitches, wounds, or scars
    • Resume work from the next day
    • Fast and smooth recovery
    • No need for daily dressing
